Clinical records

An electronic health record set up the way GPs consult

Notes, vitals, allergies, diagnoses, screening tools and letters sit in one patient record, so everything you need is on screen during the consult and nothing is captured twice.

In the consult

Everything on one screen

The record shows what matters for this visit before you start typing.

Consult notes

Write the note yourself, start from a template, or ask for an AI first draft to review.

  • Side panel flags for LNMP, contraception and last Pap smear or mammogram
  • Today’s out-of-range vitals tagged
  • Templates and the patient’s own symptom check insert at your cursor
  • History timeline shows each day’s ICD-10 codes

Vitals and allergies

Record what you measured and see how it has changed.

  • Trend graph for any vital sign
  • Blank fields never erase an earlier value
  • Medication allergies warn when a related medicine is prescribed or dispensed
  • Allergies and critical events show on the patient summary everywhere

ICD-10 diagnosis

Codes are added as you click, with no separate save.

  • Reuse every diagnosis from an earlier visit in one click
  • Add a patient’s coded chronic conditions in one click
  • First clinical code becomes the primary diagnosis
  • A claim can’t be sent until the invoice has a diagnosis code
Screening tools

Validated questionnaires, scored for you

Complete a questionnaire with the patient in the room, or send it to their phone. Either way the score is filed into today’s notes.

  • Seven built in: PHQ-9, GAD-7, EPDS, HAM-A, IPSS, IIEF-5 and PAID-5, with nothing to set up.
  • Live scoring as you answer in the room.
  • Send to the patient as a QR code, link or message. They complete it at home and the result comes back to the record.
  • Growth charts and pregnancy wheel for children and antenatal patients.
Documents

Letters and forms from the record

Create the document where the information already is, then print, email or send it on WhatsApp.

  • Sick notes and family responsibility leave letters, with quick date buttons and a full history.
  • Referral letters with saved provider contacts and the clinical history you choose to attach.
  • Consent forms signed by the patient on their phone or on your device, then counter-signed by you and filed as a PDF.
  • Attachments such as referral replies and scanned reports, checked by file content and sent again from the list.
Over time

A record that follows the patient

The big moments and the small tests stay visible, so the next consult starts from the full picture.

  • Critical events, such as an operation or a bereavement, kept on a timeline that shows across the app.
  • Point-of-care tests (finger-prick, rapid tests, urine dipstick) stored with the patient’s lab results.
  • Follow-up dates with a reason, so the patient is reminded when it’s time to come back.
  • Audit trail: entries keep who added them and when.

Questions

Clinical records: common questions

Can I use templates for my notes?

Yes. A template inserts at your cursor rather than replacing what you’ve written, so you can combine a template with the patient’s own symptom check in one note.

Which screening questionnaires are included?

PHQ-9, GAD-7, EPDS, HAM-A, IPSS, IIEF-5 and PAID-5 are built in. You can complete them in the room or send them to the patient’s phone, and the score is filed into the notes.

Does the AI write notes for me?

Only if you ask it to. Draft with Clini writes a first draft from what is already recorded and shows it to you. It never saves anything on its own, and you review and sign the note.

Can patients sign consent forms on their phone?

Yes. The patient signs on their phone or on a device in front of you, you counter-sign in Cliniweb, and the final PDF with both signatures is filed to the record.

Does it work on a tablet?

Yes. Cliniweb runs in the web browser on a desktop, laptop, tablet or phone, so there is nothing to install.
